The man on all fours is Seth, the third child of Adam and Eve, from the book of Genesis. His father, Adam, is behind him, and of course that's Eve on the right-hand side. The large, blocky, face is God's.
This is all, of course, a metaphor, and is in no way a realistic drawing. For starters, all characters are depicted as white, when clearly, they were originally very, very black.
